The Hmong HCBS Access and Equity Program for Hmong Families with Disabilities
UHD’s Hmong HCBS Access and Equity Program supports members of the Hmong disability community by connecting them with qualified Home and Community-Based Services (HCBS) providers who deliver culturally responsive care in Hennepin, Ramsey, and Washington Counties, empowering them to access the services they need to remain in their homes and thrive in the community.
What are Home and Community-Based Services (HCBS) Waiver programs?
The HCBS waiver programs are Medicaid-funded initiatives that allow the State of Minnesota to put together a range of services to help seniors and people with disabilities to get the care and support they need to live independently and remain in their homes and communities.
Primary Minnesota programs include the Alternative Care (AC), Brain Injury (BI), Community Alternative Care (CAC), Community Access for Disability Inclusion (CADI), Developmental Disability (DD) waiver, and Elderly Waiver (EW).
Eligibility for Waiver Programs and Services
- Minnesota resident
- Eligible for Medicaid/Medical Assistance
- Certified disabled (SSA, SMRT)
- Choose to live at home or in the community
- Need a level of care usually provided in a facility (for Waiver service)
Services You May Be Eligible For
HCBS programs provide both medical and non-medical support, such as:
- Case management and service coordination
- Personal care and home health aides
- Skilled nursing and therapy services
- Adult day health programs
- Respite care
- Residential and day habilitation
- Medical supplies and equipment
